Title: Kokoschka AKX returned minor issue

if you give Kokoschka his rifle back, he does not equip it so if attacked he uses his fists.

Maybe it's to allow you to pickpocket it back?

Yes, it's unequipped so that you could pickpocket it if you have no heart. I've done some tweaking, so hopefully he'll start equipping it when needed.
