Perception check for orange box sequences mixes up 1st and 2nd notes (from low to high).

Hmmm, mind if I see your save? Save can be found under C:\Users\[User name]\Documents\My Games\Underrail\Saves. You can upload it to Google/One Drive, Dropbox or any other cloud service of your liking and post the link here.
