Bugs / Scrappers found "wormhole" without me

I had finished the first two Scrapper quests (card/disc) and said nothing further to Eddie/accepted no other quests. I had finished all the Black Eels quests leading up to the attack, including getting SGS to agree to help, but the next time I talked to Silas, he told me the Scrappers found the wormhole, and the quest is failed in my notes. Talking to Eddie, he tells me "I was going to ask you to find the wormhole, but it looks like you already took care of that." Not sure if this is a bug, but in 7 or 8 playthroughs, I've never had this happen (I always side Eels).

During combat, I can use bandages.

That's it. I can always use them in every combat, indefinitely.
