'Keycard required' appears twice, seems to be the case with the other Electronic Locks in the facility as well.

That's strange... In my case, I never encountered any problems with my current character, who asked for nothing when he returned the ring.
I'm not sure how else I can help. You may want to save a backup save of the playthrough where it happened, then ask the developers about this. Post it in the 'Bugs' section, the more details the better. Or consider direct messaging them.

I do remember going through the quest normally till completion, recently.
I suspect...
Do you have the ring in your inventory when you spoke to Chester Royce? Obviously you need it with you to show it to him.

(3) I feel this: The Praetorians are going to wipe the floor with the Faceless if they dare to try to move into the city. There's a lot of bad stuff I can say about Praetorian Security, but in these type of situations I have good faith in their enforcers' abilities.

Despite possessing the 'Keycard' which can unlock the doors at the Coretech Research facility, the shortcut door which bypasses the corridor with the crawlers cannot be unlocked with the 'Keycard'.
