If you fix Matriuss' device but later kill him and the Praetorian Sniper nearby, you can loot both the broken version from sniper and fixed one from ludenloff.

When you click buttons on phone box north of fort apogee(before plugging orange box) both options automatically disconnects you from the phone when one should return you back to the button select menu

Wounded Man in Ion's missing group quest gets 'destroyed' like an object instead of dying like normal NPC(Cliff for example).

Phreak says he has a dozen more forged tickets, says he will sell 10 but you receive 8.

Not sure if this is intended or not, but you cannot tell the Faceless about their missing item if you have talked to Cornell about the Cube during Phreak questline
