Technically, you also need to learn the name of Cornell's gang, the Acid Hunters, for the quest to trigger, which Buzzer mentions once you ask him if he knows where Cornell is heading. My guess is that you probably asked him the first few questions and missed that last one. I did a quick check in his dialogue file just in case, but everything seems alright.

That junk pile is bugged, so it pretty much swallowed the figurine. Fixed it for the next patch. As for the item itself, I've sent you a PM.

To everyone else - do not leave your important items in junk piles, barrels and similar, since there's still more bugged containers out there.

Olivia's quest is a separate entry in the quest notes, and that one should be marked as completed after you report it to her. On the other hand, there is a bug where Nicolas' quest does not fail after you kill him, as you can no longer complete it anymore, and it will be fixed in the next patch.

I was about to post the same quest. I killed Nicolas by sabotaging the console. Agent showed up to confirm. However I get no new dialogue options when I talk to Olivia.
