At least increase the spawn chance. In 500 hours of playing i never seen him once.

1700 hundred and 0 spawns. I’ve tried for hours trying to find him too

He won't spawn at all on some runs.  It's random at game start.

Damn. Just started hard difficulty for the first time and was kinda looking forward to running into that guy. Oh well, guess I'll just focus on other stuff.

The first time I have ever seen a two-headed mutant was during this playthrough after almost 700 hours in the game. Way too rare, they should be a guaranteed spawn, although I seriously suggest they should be post-Depot A only because fighting the equivalent of a hunchback mutant when you're at best level 9 is too painful even for DOMINATING.
