I haven't seen this raised in a while so I thought I'd summarize what I know about this issue in this post. The issue is:
Having visibility from A to B doesn't necessarily mean having visibility from B to A. This always or almost always happens when A and/or B is near a corner.

- Being able to melee a target (or being attacked in melee) but not to shoot it without moving first.
- Combat is initiated by an NPC and you win the initiative but you have to move to actually see who spotted you first.
- Character A being in melee range of character B but not vice versa. May be related to A using a spear which has extended range.
- Waiting for a patrolling guard to disappear from sight so you can steal something, just to be caught.
- Being seen in stealth while you shouldn't be or being able to see a character who is clearly in range but cannot see you.
Attached are a couple of spots where the latter can be tested consistently. #1 Foundry. I can see Leonie (and steal from the containers) while she cannot see me. #2 Camp Hathor. Standing here and messing with the safe will get you spotted by Boyd who is just around the corner and out of sight.
