Based on your description it is most likely that for some reason the game did not record correctly the fact that you restealthed during combat (for example you failed to actually get out of sight from the enemy).

Bugs / Heavy Duty ending slides problem (again)

I killed Cognator and got the ending slide where he's still alive.

This slide: "And so you severed the command chain right at the very core." only triggers if the player has killed Cognator without talking to him first (so talking with him and using "I'm here for revenge *Attack!*" dialogue option causes him to magically survive off-screen).

It seems that doctor m is mentioned in the kharon ending slide even if he's dead (scenario where player for some reason only killed him back in black sea).

Best outcome would be probably if he had the same response as when player mentions being a tchortist with slightly modified text

You cannot talk with Doctor M about donating meds when you first speak with him even if you talked with Wesley already. Introduction dialogue will always play out the same as if player is looking for Wesley.

Bugs / JKK Vivian quest comm device and generator

You cannot ask Vivian for help with the generator immediately even if you fixed everything, have all the parts and are next to the generator when picking 'I checked the hydraulic generator...' option. This option ends with player turning off the comm for no reason even if you can fix it right now.
Player has to manually turn it on again for the option to get it fixed to appear.

I wish I could do the same, but even if I click "Skip Tutorial," there's no way I can play. It just keeps blanking out right after Hadrian's scene right at the start. I literally can't play far enough to even get a game save. Are you able to actually get beyond there?
If the game is crashing for you right after tutorial, it might be antivirus related. If you haven't tried, check if turning it off for a moment or putting the game in the exceptions of your antivirus fixes the issue.

Unsure if this is important but if you attack Holloway, some of his soldiers stay neutral at the start of the fight if you win the initiative roll until you attack one of them.
